To improve the efficiency and accuracy of patient referrals, I led the development of a standardized referral package within the EMR, focusing on Home Health, Durable Medical Equipment (DME), and Home Infusion services. This initiative aimed to reduce delays, improve collaboration, and ensure smoother transitions for patients requiring post-acute care services.

Key Steps Taken:

  • Collaborated with key stakeholders & SMEs to define referral criteria, ensuring all necessary documentation was captured from the start.

  • Developed structured EMR templates tailored to specific referral types, improving accuracy and compliance with insurance requirements.

  • Created a comprehensive checklist for discharge planners to standardize the referral process and reduce inconsistencies.

  • Conducted cross-functional training sessions for physicians, nurses, and case managers to enhance understanding of the referral process and proper documentation techniques.

Impact:

  • Reduced hospital length of stay by transitioning from a reactive to a proactive referral process.

  • Improved documentation accuracy to enhance insurance approvals and minimize reimbursement issues.
